Terrible service. I mailed forms to them but they claimed they didn’t receive it. I told them I also filled out the form on their website. They said the online form is a waste of time they shouldn’t have it online if it is a waste of time.Only other method of submitting anything is via fax. I asked the woman on the phone to resend the forms via mail and asked her if she has my address (because I’ve put it on the online form and she’s found our info in their system). She snapped at me and said she’s been on the phone with me for too long.This is an agency that’s supposed to help people work disability. They should have more patience and apprehension that they exist to provide service to a vulnerable population. I called for my son who has disability and was able to stand my ground but I know for sure of my son called, this woman would have crushed his confidence or any sense of possibility of getting any help. Horrible place.

I called them three weeks ago they told me they are going to mail the forms. I called them again the following week since i have not gotten my forms to fill up. This is just a waste of time.

This state agency needs to be investigated and falsified documents I have a job coach who placed me in a job I didn’t want under a pretense of getting me into a school she never contacted or visited my employer since I been going to see her.
